Abstrakt     Although the kidney transplant recipient has reached a permanent solution, he remains under permanent care of his specialist nephrologist as will as his family physician. The consequences of imunosupressive therapy, the rejection of transplant, infection, toxic side - effect of therapy, hypertension, cardiovascular complications, hepatitis and increased incidence of malignant tumors demand a close follow - up of the health status of transplanted patients. The life style and living environment changes, the altered working ability and a precise therapeutic plan demand a complete adherence to therapy - not only from the patient, but also from his family. The patient - doctor relationship should give patient enough autonomy and include him in decision making, while the doctor is an expert adviser. The primary care doctor is a coordinator between the patient, his family and the specialist nephrologist. Permanent education of patients, their families and of primary care physicians can further improve the outcome of therapy of transplanted patients.
