Abstrakt     Enlarged lymph nodes (lymphadenopathy) and an enlarged spleen (splenomegaly) are frequent clinical signs, which may present a diagnostic problem and a therapeutic challenge. Our paper describes the features of the peripheral lymphatic system including the lymph nodes and the spleen. It also lists those diseases in which lymphadenopathy (LA) and splenomegaly (SM) occur. LA most often implies reactively enlarged lymph nodes, the cause can be explainedby the clinical picture and the course of disease. In such cases additional investigations are not necessary. However, enlarged lymph nodes, which do not return to normal within one month, are a different case. These should be defined causally. SM indicates a palpable enlarged spleen reaching under the left costal arch. It commonly means a pathologic condition, although a moderately enlarged spleen can be found in about 3% of the healthy young population. The most frequent cause of SM is a disease of hematopoietic organs, but an enlarged spleen is often present also in infections, chronic infammations and in portal hypertension. Other causes are rarely seen at our institution.
