Abstrakt     Background: Many positive effects of using treadmill training in the rehabilitation of stroke patients are known. There is, however, a lack of evidence that would promote its use for improvement of walking pattern when compared to other treatment techniques. Purpose: To compare the spatio-temporal and lower limb kinematic characteristics of walking on a treadmill and overground. Methods: Ten independently ambulant chronic stroke subjects were included in the gait analysis. A Vicon® motion analysis system was used to collect spatio-temporal and kinematic data during walking overground and on the treadmill. Results: Walking on the treadmill demonstrated lower cadence (p<0.0001), and longer step times of the nonhemiplegic (p<0.001) and hemiplegic (p<0.01) limb. Compared to overground walking, absolute stance times of both limbs (both p< 0.01) and absolute double support time (p<0.001) were longer on the treadmill as were relative stance time (p<0.05) and relative double support time (p<0.001). Relative single support time of the nonhemiglegic limb (p<0.05) was shorter on the treadmill. Kinematic data showed greater flexion of the nonhemiplegic knee and hip at initial contact (p<0.05 and p<0.01), and less hip extension of the hemiplegic limb (p<0.05), with greater maximal hip flexion of both limbs during treadmill walking. Maximal ankle plantarflexion and knee extension of the hemiplegic limb occurred later in the gait cycle (both p<0.05) on the treadmill. Conclusions: Walking on a treadmill results in different spatio-temporal and kinematic gait characteristics from walking overground. This needs to be taken into account when using treadmill training to improve gait.
