Abstrakt		In this article we report the case of a patient who has been admitted to a local hospital because of an myocardial infarction. Electrocardiogram predicted an important three-vessel disease. After the clinical picture of pulmonary edema was resolved, he was transfered to the clinical centre for invasive diagnostics. A by-pass surgery was performed, because urgent coronarography has shown an occlusion of the left main coronary artery and stenosis of the right coronary artery. One year after the myocardial infarction the patient is cardially compensated and classified as NYHA 2 category. It is very important to recognize high risk patients with acute coronary syndrome in the prehospital assessment, where the ECG is a very useful instrument.
