Abstrakt     The postinfarction ventricular septal defect is a rare, but life-threatening complication of an acute myocardial infarction. It occurs mostly in patients in which reprefusion was not achieved in the acute phase of myocardial infarction. The treatment is surgical and the mortality rate is high: We present a case of a 77-year old patient with prolonged chest pain four days before admission to the hospital. Two days later he became febrile and on the admission day dispnea and epigastric pain developed. Clinically, the postinfarction ventricular septal defect was suspected. An urgent cardiac catheterization revealed coronary artery disease and large ventricular septal defect, the diagnosis was additionally confirmed with the transesophageal echocardiogram. An urgent surgery was performed and the patient died three days after the surgery. The autopsy showed large myočardial infarction.
