Abstrakt     The symptoms of depressive syndromes are more frequent in the physically il! than in the genera! population. Depression in the physically ill affects their experiencing physical illness and their compliance and influences the quality of life and social functioning. Between depressive disorders and physical illnesses there may be a number of potential relationships. These can range from a purely coincidental occurrence (comorbidity) to a direct causul role of pathophysiologic and psychologic factors - whether physical illness or drug treatment - in the development of depressive disorder. The authors present a case of a depressive patient with end-stage renal disease on hemodialysis to point to difficulties in recognizing depression in the physically ill and to dilemmas of differential diagnosis the authors had to face when trying to classify a concrete disorder according to the International classification of diseases ICD-10.
