Abstrakt     Recent studies have shown that stromal reaction in cancer has a more important diagnostic and prognostic significance than previously thought. The aim of our study was therefore to analyze the distribution and immunohistochemical phenotype of stromal cells in epithelial hyperplastic lesions (EHL) and squamous carcinoma (SC) of the larynx and pharynx, and to establish their diagnostic and prognostic significance. We investigated samples of 42 resected larynxes with SC, 12 laryngeal biopsies of EHL and SC and 6 samples of normal adult and fetal laryngeal mucosa. Immunohistochemistry was performed using antibodies against a-smooth muscle actin (SMA), CD34, CD31, CD117, TGF(31 and TGFbeta1 receptor. RNA in situ hybridization was performed using specific oligonucleotides for TGFbeta1. The stroma of EHL contained scattered CD34-positive cells, but there were no SMA-positive myofibroblasts. In contrast, the stroma of SC contained SMA-positive myofibroblasts but there were no CD34-positive cells. Some myofibroblasts stained for CD117. TGFbeta1 and its receptor were present in tumor cells as well as in inflammatory cells and myofibroblasts in the stroma of SC. Stromal reaction at the invasive front and in peritumoral space was dependent of the pattern of tumor growth at the invasive front (infiltrative, expansive, mixed). Only association between the intensity of inflammatory reaction in the peritumoral zone and the tumor diameter was statistically significant (p<0.05). Our results show that disappearanec of CD34-positive cells and appearance of SMA-positive myofibroblasts in the stroma is associated with transformation of EHL to SC of the larynx and pharynx.
