Abstrakt     Our study was carried out in 55 patients with severe form of acute pancreatitis, treated in the University Department of Gastroenterological Surgery during the years 1986-1990, with the aim of establishing the diagnostic and prognostic value of native CT examination. The results of CT in individual patients were compared with the findings obtained on subsequent surgery. Owing to advanced disease and consequential complications, in some patients the investigation had to be repeated. Altogether 73 procedures followed by surgery have been considered. The introductory chapter reviews the existing knowledge on AP etiology and diagnosis, with special emphasis on noninvasive diagnostic methods. In our series of 55 patients with severe form of AP, the course of disease was uneventful in 11 patients whereas the remaining 44 presented with compiications. Cure was achieved in 32 patients, and in 23 cases the disease ended with a fatal outcome. Our assessment of the diagnostic value of native CT investigation was based on the comparison of CT results with surgical findings. The size, density and localisation of infiltrate were evaluated. Intrapancreatic infiltrate location was found by 71 examinations, extrapancreatic in the omental bursa by 67 and in the fatty tissue by 69 examinations. The resolution of infiltrates smailer than 3 and 5 cm respectively, situated intra and extrapancreatically in the fatty tissue, was found to be good, whereas better imaging of infiltrates exceeding 5 cm of size was seen in the omental bursa. In density measurements, the highest number of intra- and extrahepatic findings fell within the range of values applicable to necroses and abscesses.(trunc.)
