Abstract     Background: An unstable urinary bladder causes micturition problems, recurrent urinary tract infections, while a bladder-urethral reflux is also possible. The idiopathic form of an unstable bladder probably results from a delayed maturing of the central nervous system. Methods: In 55 girls aged 5 to 15 years (on average 9.5 years) who had voiding abnormalities and/or urinary tract infections, urodynamic measurements of the lower urinary tract were carried out: pressure in the bladder and abdominal cavity, electromiography of the muscles of the pelvic base, urine flow and electrical conductivity of the urethra. Results: In 76 per cent of the girls an unstable bladder was present in 40 per cent during filling of the bladder and in 60 per cent at fhe end of cystometry, with or without provocation. In 45 per cent of the girls a pathological flow of urine was found due to a hyperactive urethra. In 65 per cent of the girls post-voiding contractions of the detrusor were detected. The presence of daily enuresis significantly increased the freguency of an unstable bladder. Conclusions: An idiopathic unstable bladder is a very freguent abnormality in pediatric urology. Urodynamic meosurements make an evaluotion of the abnormality, possible on the basis of which the correct treatment for its clinical conseguences is selected.
