Abstract     In order to study separately the effect of an acute increase of insulin and triglycerides on tissue plasminogen activator (t-PA) and its inhibitor (PAI-1), ten healty volunteers, aged 25 to 44 years, were randomly assigned to a meal with increased amount of fat (3900 kJ), a meal consisting almost entirely of carbohydrates with an equal caloric value, and to fasting. The first blood sample was drawn in the morning before the meal and then 1, 2, 4 and 8 hours after the meal. The same time schedule was used during fasting. The following parameters were determined: serum levels of glucose, insulin, total cholesterol, HDL cholesterol and triglycerides, euglobulin clot lysis time (ECLT), t-PA antigen, tPA activity, PAI-1 antigen and PAI activity. During fasting the fibrinolytic parameters varied according the well-known diurnal rhythm (a steady decrease in t-PA and PAI-1 antigen and PAI activity, and an increase in t-PA activity and ECLT). An increase of triglycerides and the moderate increase of glucose and insulin occured after the high fat meal while the fibrinolytic parameters showed no departure from their diurnal pattern. After the high carbohydrate meal there was a much higher increase of glucose and insulin 1 hour later. Two hours after the meal PAI-1 antigen and actvity significantly increased. Both peaks occured at least one hour after the glucose and insulin peak. It can be concluded that fat meal and the resulting increase of triglycerides does not influence PAI-1 and other studied fibrinolytic parameters, while cells and due to the anabolic influence of insulin. carbohydrate meal and resulting increase of insulin are causally related to the increased PAI-1.
