Abstract     A conservative surgical approach for early invasive atage IA cervical carcinoma was adopted in 1981, when a scoring system based on the evaluation of morphological criteria was introduced in our Department. Results with the different treatment modalities of 339 cervical carcinomas at stage IA operated in our Department between 1981 and 2002 are presented. In the first period (198188), 36,1% of cases were treated by conization, 33,7% by simple and 30,2% by radical hysterectomy. In the last period (1994-2002), conization was performed in 71% of cases, simple hysterectomy in 25%, hysterectomy with pelvic lymphadenectomy in 4%, and there were no cases of radical hysterectomy. Our experience indicates that conšervative management of stage IA cervical cancer is indicated and safe when the exact evaluation of tumour extension and surgical margins of the cone are assured.
