Abstract     The prevalence of gallstones in Europe is 10%. They are symptomatic in 30%. Gallstone ileus is their rare complication, as a consequence of migration of gallstones from inflammed gallblader to duodenum and further to jejunum and ileum, where they block lumen and cause ileus. Treatment should be tailored to each individual patient. Optimal surgical treatment is one stage procedure: cholecystectomy, fistula repair and enterotomy with removal of impacted stone. We prefer only enterotomy with stone removal in high risk patient. Early diagnosis and prompt therapy may reduce morbidity.
