Abstract     Background. Cholecystectomy is amongst most common elective operations in abdominal cavity in western world. According to some series, about 10 to 30 percent of cholecystectomies are performed in patients with acute cholecystitis. Laparoscopic cholecystectomy has become golden standard for elective treatment of symptomatic gallbladder stones soon after its wide use more than decade ago. The role of laparoscopic cholecystectomy in treatment of patients with acute cholecystitis is almost routine but open cholecystectomy still plays an important role. Biliary injuries are most serious complications of cholecystectomy. Patients and methods. Department of abdominal surgery BPD was in 2002 24 hours a-day service for abdominal emergencies for central Slovenia and Ljubljana, which covers about one million inhabitants, for 24 days a-month. There were 80 beds in 2 wards and ICU and 2 operation theatres. There were 12 surgeons, 10 surgeons were skilled in laparoscopic surgery. There were 773 patients with gallbladder stones operated on in 2002. Principles of safety in laparoscopic cholecystectomy were followed. Results. In 2002 there were 504(65,2%) laparoscopic cholecystectomies. There were 204(26,4%) patiens with acute cholecystitis operated on, of them 63(30,9%) patients were operated on laparoscopic. There were 68(13,5%) conversions in 504 laparoscopic cholecystectomies. There were no biliary injuries reports in 2002. Conclusions. Laparoscopic cholecystectomy has become wide used also for treatment of patients with acute cholecystitis. In 2002 there were relative low ratio of laparoscopic cholecystectomies and relative high ratio of conversions to open operations in patients operated on because of symptomatic gallbladder stones in department of abdominal surgery BPD.
