Abstract     The number of patients who require anticoagulation therapy is increasing. From a narrow range of patients with venous thrombosis, pulmonary embolisms, artificial heart valves and some cardiac defects, the indications have been expanded to include those with atrial fibrillation without accompanying valve damage. There are many patients with atrial fibrillation (AF) in the general population; its incidence after the age of 60 is said to be 5% and it increases with age. One of the complications of AF are brain embolisms. The annual incidence of brain embolisms in patients with AF ranges between 5 and 7%, but increases to 18% in the presence of other risk factors, such as old age, previous cerebrovascular accident or transitory ischemic attacks, diabetes, arterial hypertension, coronary heart disease and heart failure. Over one half of patients with thromboembolic cerebrovascular accident suffers from resulting severe neurological impairment, which leads to permanent disability and reduce the quality of life.
Summary     Antikoagulacijsko zdravljenje s kumarini zmanjšuje tveganje za možgansko kap pri visoko ogroženih bolnikih. Zaradi porasta bolnikov, ki potrebujejo tovrstno zdravljenje, se je del vodenja antikoagulacijske zaščite prenesel iz sekundarne ravni na primarno raven zdravstvenega varstva. V retrospektivni raziskavi smo pri 374 bolnikih, ki so bili vodeni s pomočjo računalniškega Thromboprograma ocenjevali zaplete zdravljenja. V dvoletnem obdobju smo zasledili le nekaj zapletov zdravljenja, ki pa so se zadovoljivo razrešili, smrtnih primerov ni bilo. Bolniki so bili vodeni varno, organizacija mreže protikoagulacijskih ambulant na primarni ravni je smiselna in upravičena.
