Abstract     Erythrocytosis is characterized by pathologic elevation of red blood cell mass. The normal red cell mass at sea level is 26 to 32 mL per kilogram of body weight for men and 23 to 29 mL per kilogram for women. Erythrocytosis can be classified by the underlying pathophysiologic mechanisms as primary or secondary erythrocytosis. Primary erythrocytosis represents autonomous proliferation of erythroid progenitors as a result of an intrinsic cellular defect. Secondary erythrocytosis is a heterogeneous group of disorders where erythrocytosis is because of either a decreased tissue oxygenation or the physiologically inappropriate production of erythropoietin. Erythrocytosis is initially considered on the basis of elevated hematocrit. A man or a woman with hematocrit level of 0.60 or higher or 0.57 or higher, respectively, virtually always has a true increase in red blood cell mass (i. e., primary or secondary erythrocytosis). If patient's hematocrit is below these values but above normal, a red blood cell mass study should be done. With this we identify patients having elevation of hematocrit because of a plasma volume contraction (spurious erythrocytosis).
Summary     O eritroctozi govorimo, kadar je masa eritrocitov v krvnem obtoku patološko povečana. Normalna masa eritrocitov v krvnem obtoku pri nadmorski višini 0 m, izražena kot celotni volumen eritrocitov, je za moške 26-32 mL, za ženske pa 23-29 mL na kilogram telesne teže. Glede na način nastanka razlikujemo primarno in sekundarno eritrocitozo. Primarna eritrocitoza je posledica avtonomne prekomerne tvorbe eritrocitov zaradi napake v krvotvorni matični celici. Sekundarna eritrocitoza nastane pri različnih bolezenskih stanjih kot fiziološki odgovor na hipoksijo ali zaradi fiziološko neustreznega izločanja eritropoetina. V vsakdanjem kliničnem delu pomislimo na eritrocitozo, če ugotovimo povečano vrednost volumna stisnjenih eritrocitov (hematokrit). Moški s hematokritom 0,60 ali več in ženske s hematokritom 0,57 ali več imajo skoraj vedno povečano maso eritrocitov v krvnem obtoku (eritrocitozo). Pri osebah z volumnom stisnjenih eritrocitov (VSE) pod temi vrednostmi, vendar nad zgornjo normalno vrednostjo, gre lahko za lažno eritrocitozo zaradi zmanjšanega volumna plazme, zato je v takih primerih potrebno določiti maso eritrocitov v krvnem obtoku.
