Abstract     Although the benefits of early fracture fixation are well documented, questions remain concerning the optimal timing and the method of treatment in patients with multiple injuries. Early and if possible stabile and definitive procedures has been shown to reduce the morbidity and mortality of patients and risk of another procedures and complications. They are but often of long duration and high demands what all brings the external fixation as a main decission in the primary management, being converted because of the advantages to the internal fixation as soon as possible. If patient's parameters do not stabilize, the external fixator used as an optimal control damage solution in the highly risked patients, can be used for definitive fixation.
