Abstract     Background: Serious blunt multiple trauma has the potential of causing a variety of distinct patterns of injury. Many, if not most victims have one or more fractures of long bone or the pelvic ring in addition to other injuries. The most appropriate timing and the method of operative treatment of fractures are still controversial. Methods: Review of the recent literature. The principles determined are based on the treatement experience of over 300 polytraumatized patients collected in the polytrauma registry in last 8 years. Conclusions: Many clinical studies have identified a number of apparent advantages to early definitive treatment of fractures in a patient who has multiple injuries. The ability of such patient to tolerate additional operation immediately after a life-saving one varies. The surgical team needs to know when to quit. Short, simple, relatively bloodless procedures should be planned. Simple halfpin external fixation provides excellent stability for diaphyseal and pelvic fractures. Nonurgent reconstruction is completed as the patient's general condition allows. These critically ill patients are best treated in trauma centers that are equiped and trained to manage these complex problems.
