Abstract     Background: Proper vascular access is of utmost importance for adequate haemodialysis and the long-term survival of chronic haemodialysis patients. The native arteriovenous fistula is preferred. The success rate of arteriovenous fistula construction mainly depends on the artery and vein status upon surgery. Hence, every effort to improve artery and vein status before an operation is welcome. The purpose of Study 1 was to investigate the effect of handgrip training on the forearm arteries and veins and on endotheliumdependent vasodilatation in patients with end-stage renal disease on haemodialvsis. The purpose of Study 2 was to investigate the effect of intermittent obliteration of forearm venous circulation by tourniquet on the forearm veins. The purpose of Study 3 was to investigate the additive effect of handgrip training and intermittent obliteration of forearm venous circulation on the forearm arteries and veins. Methods: All patients included in our studies had end-stage renal failure and were on clwonic haemodialysis. Study 1 - Fourteen patients were subjected to regular handgrip exercise. The clinical status of arteries and veins was assessed during the course of training. The following parameters were measured at the beginning of our study, and after four and eight weeks of training: forearm circumference, maximal handgrip strength, radial artery parameters before and after hyperemic test, brachial artery parameters, including endotheliumdependent and endothelium-independent vasodilatation and intima-media thickness, and vein diameter before and after placement of the tourniquet using ultrasound and duplex Doppler scanning.
