Abstract     In our research we evaluated changes in vaginal microbial flora influence the frequency of complications after conization. We also searched for other risk factors for these complications, including different operative techniques used for conization at our department. We retrospectively collected data of patients treated by conization at our department between 1993 and 2001. We were interested in the results of microbial cervical smears for Chlamydia trachomatis and other genital pathogens, age, vaginal discharge before operative therapy, conization technique (1. Cold knife conization with modified Sturmdorf stich, 2. Cold knife conization with electrocoagulation of conus bed, 3. Large-loop excision of the transformation zone) and antibiotic treatment during or after operative procedure. Then we evaluated how these factors influence morbidity after conization in form of increased body temperature, need for revision and problems caused by bleeding, pain and discharge which the patients recorded after the operative procedure. We established, that pathologic changes in vaginal microbial flora and the presence of genital pathogens bear no influence on morbidity after conization. The major risk factors for higher morbidity after conization in our study were patient age and genital discharge before the operative procedure. We conclude that elderly patients with genital discharge before the operative procedure are at higher risk for morbidity after conization. Among the operative techniques performed in our patients, cold knife conization with modified Strumdorf stich was the one with the lowest morbidity after conization. We therefore believe this should be the technique of choice in high risk patients mentioned above.
