Abstract     Background and methods. The author presents the treatment of lumbar radicular compressive syndromes, the clinical picture before and after operation, the successfulness of microsurgical procedure according to the technique by Caspar in the series of hundred patients. The follow up of patients was evaluated from one to three years postoperatively, the average time being 17 months. Results. The results show excellent outcome in 67 per cent , good outcome in 28 per cent , fair in 1 per cent and poor in 3 per cent of patients. One patient died one month after the operation becuuse of the other reasons and was not included in the final assessment. The total result was thus satisfactory in 95 per cent . Conclusions. The results of microsurgical approach showed that the efficiency of such an operation in this series can be compared with similar results elsewhere according to the international criteria (88 per cent - 98 per cent ). The successfulness of microsurgical method is due to the gentle tissue handling, especially with respect to nerve structures within the spinal canal us well us considerably smaller operative injury of the muscular and bony tissue. This is achieved by better visibility and field magnification with the use of microscope.
