Abstract     Background. The first successful free transfer of a combined dermal-fascial free flap in man was carried out in Maribor Hospital in 1989. Methods. The operative intervention was made in phases, comprising removal of devitalized tissue, production of a new arterial curve of the hand with vein transplantation, the insertion of live transplants of the calf nerve in place of the joint finger nerves, the lifting and transfer of the free flap into the new bed and revision of the microvascular repair. Results. The free flap survived as a whole, and after one month the patient performed easy work. After 18 months the flexible contractures had somewhat improved. The partial reinnervation reduced the loss of sensibility, while the patient could carry out most of his farming activities. Conclusions. In spite of injury to the sinews and minor muscles of the palm and the scar contractures, the use of functional loss of the hand did not exceed 50 per cent . The free flap made it possible to carry out most of the necessary reconstructive interventions and reduced the number of corrective ones.
