Abstract     The "Monorail" method presents an alternative way of bone transport and elongation. It features an intramedullary inserted locking nail and an unilateral external fixator with the capability of controlled segmental bone distraction. A case report of a 28-years old female with the spiral comminuted femoral fracture is present. After the application of the Unreamed Femoral Nail a malunion has developed with 20 mm of leg shortage and 30° external rotation. In the second operation the distal locking screws were removed the osteotomy distal to the callus was made, derotation of the bone and an unilateral external fixator was applied. In the next moth the bone was distracted 1 mm per day. Ned result is satisfactory. The patient has no pains or other troubles any more. The Monorali method is expected to be the standard method of bone transport ans elongation because of its simplicity.
