Abstract     Background. Cervical cancer (CC) is the third most common cancer in women of Slovenian Coast with the incidence higher than the national one. The aim of the analysis was to estimate: the qualify of screening, the role of cytologic findings in detection of early invasive CC and the rate of cytologic failure. Methods. The analysis included 37 cases of CC with histologic diagnoses being confirmed af the Department of Cytology and Pathology (General Hospital Izola) from 1999 fo 2003. All previous PAP smears were reviewed. Histologic findings and clinical data were gathered from our archive and medical files of women with CC. Results. 28 cytologically examined women wifh CC had suspect (10) or positive (18) smear finding. Diagnostic accuracy rates for invasive and microinvasive CC were 87,5% and 33,3%, respectively. 86% of women with CC visited the gynaecologist irregularly. At least 62% of CC were classified as FIGO stage 1. Most of the women with CC were in the 4049 and 60-64 age group. The average proportion between CC and CIN III was 1:10. The crude incidence of CC was 18,4/100.000. Conclusions. The screening for CC on Slovenian Coast showed high sensitivity. The cytologic finding had crucial role in diagnostic procedure for 68% of cytologically examined women. As a result, 62% of CC was in early stage. The failure rate in smears taken before CC was recognised was 9,5% in cytologic interpretation and 4,7% in smear taking.
