Abstract     Introduction: Only few reports can be found in the literature on radiologic imaging of normal pleural fluid in healthy individuals, using lateral decubitus chest X - ray, but none using ultrasonography. Based on results of our own scientific work in the field of small pleural effusion diagnostic imaging we hypothetically presumed that normal pleural fluid could be shown with ultrasonography. We also tried to determine which diagnostic modality is more accurate in detection of normal pleural fluid ultrasonography or lateral decubitus expiratory chest X-ray examination. Participants and methods: Chest sonography was performed in 106 healthy volunteers, searching for anechoic layer between parietal and visceral pleura, first in experimental lateral decubitus position, then leaning on the elbow and finally in a sitting.position. Both pleural spaces were examined. At least 2 mm thick anechoic layer at examination in lateral decubitus position represents positive fmding. After sonography, chest x - ray examination in erect posteroanterior and expiratory lateral decubitus projections were performed. At least 2 mm thick horizontal layer next to ribs was defined to represent positive result. Sonography follow-up examination was done few weeks after baseline examination, to prove that initially examined volunteers were indeed healthy. In addition, 47 healthy pregnant women were examined ultrasonographically, using the same procedure. The aim of this part of the study was to cover the population, which undergo frequent ultrasonography examination within short period of time (pregnancy). Results: The fluid layer of typical wedge-shaped appearance was visible in the pleural space of 28/106 healthy volunteers (26.4%) at the event of initial examination. In 17, the pleural fluid was visible on both sides, while in 11 only unilaterally. Follow up results were positive in 26 (24.5%) volunteers. (Abstract truncated at 2000 characters).
