Abstract     The family physician has a unique gate keeping position in the health care system. He coordinates the health services and has the best knowledge of patient health related problems. The majority of health problems can be dealt with in the family practice by the primary care team. Referral to the consultant when necessary is an important decision. Longer waiting lists and higher health care costs are the outcome of rising referral rates. Numerous possible causes for inappropriate referrals are listed. Better physician-physician communication, computerized data exchange and a change in the patient's and the physician's perception regarding primary care will eventually lead to a more rational referral policy.
