Abstract     The mastoid air cell system plays an important role in the physiology of middle ear function. It is also an important contributor to the pathophysiology of middle ear inflammatory diseases. Many diagnostic procedures have been designed for evaluating the mastoid air cell system. The extent of pneumatisation can be only clinically estimated by observing lateral radiographs; standard planimetry or computerised planimetry can be used. Lately, computed tomography has been used as an advanced radiographic tool for assessing mastoid pneumatisation. According to reports, postsurgical functional results also depend on the size of the mastoid cell system. Aeration of middle ear cavities appears to be the most important factor for achieving good results of tympanoplasty. A comparison was done of the pneumatisation surface of the mastoid air cell system with the functional results of type I tympanoplasty. The estimated hearing results were based on elevation of the air curve and on the residual conductive component of hearing loss at 500, 1000, 2000 and 4000 Hz. Calculation of linear regression was used for assessing the influence of mastoid pneumatisation on the functional results of type I tympanoplasty. The regression coefficients were calculated: B??????on=2.4 (p=0.04) and Bresidual=3.18 (p=0.005). The incidence of reperforations does not show a significant correlation to the extent of pneumatisation (p=0.338).
Summary     Zračni sistem mastoidnih celic je pomemben dejavnik pri fiziologiji srednjega ušesa. Vnetja srednjega ušesa zavirajo pnevmatizacijo mastoidnih celic. Opisani so številni postopki za oceno in izmero pnevmatizacije mastoida. Pnevmatizacijo mastoida lahko ocenimo s pregledom rentgenogramov senčnične kosti v stranski projekciji, možno jo je oceniti s planimetrijo in z izračunom površine pravokotnikov, ki v grobem pokrijejo področje pnevmatizacije na rentgenogramu. V zadnjem času so se razvile nove metode za oceno in izračun velikosti pnevmatizacije mastoida z digitalno obdelavo dvodimenzionalnih in tridimenzionalnih računalniških tomogramov. Predhodne raziskave so pokazale, da je operacijski izid pri zdravljenju kroničnih vnetij srednjega ušesa odvisen tudi od velikosti pnevmatizacije mastoida, saj je zračenje votline srednjega ušesa najpomembnejši dejavnik za dober uspeh timpanoplastike. V naši raziskavi smo velikost pnevmatizacije mastoida ovrednotili z računalniško planimetrijo. Funkcionalni uspeh timpanoplastik tipa I smo vrednotili z dvigom zračne krivulje po operaciji in z ostankom prevodne komponente izgube sluha. Z uporabo linearne regresije smo vrednotili vpliv površine pnevmatiziranega področja senčnične kosti na povprečni dvig zračne krivulje in na ostanek prevodne izgube sluha po operaciji. Izračunali smo regresijska koeficienta, ki sta bila Bdvig=2,4 (p=0,04) in Bostanek-3,18 (p=0,005). Uspeh operacije smo vrednotili tudi s ponovnim pojavom reperforacije, ki z izračunom logistične regresije ni bil statistično značilno odvisen od velikosti pnevmatizacije (p=0,338).
