Abstract     Chronic diseases are the major cause of morbidity, mortality and disability in modern developed and developing countries. Good follow-up and control of chronic diseases constitute an especially important part of everyday work of family physicians. A good partnership between the patient and his/her doctor is the prerequisite of successful control of chronic diseases. This is a demanding process, a long-term project that lasts throughout the patient's (and doctor's) life, from the moment of diagnosis. Therefore, this type of work has become an important part of a GP's workload, but also an important tool for quality assessment procedures, providing a solid basis for co-operation between GPs and various specialists. The article provides a few tips and guidelines for successful management of chronic diseases in family medicine.
Summary     Kronične bolezni so glavni vzrok zbolevanja, umiranja in invalidnosti sodobnega človeka, obvladovanje in dobro vodenje kroničnih nenalezljivih bolezni je zato postalo pomemben del dela družinskega zdravnika. Uspešno vodenje kronične bolezni zahteva sodelovanje obeh partnerjev bolnika in zdravnika. Proces je zahteven, dolgotrajen in dolgoročen, predstavlja dobršen del opravil družinskega zdravnika, primerno merilo kakovosti dela družinske medicine in pomembno mesto sodelovanja s specialisti. V prispevku navajamo priporočila in navodila za uspešnejše vodenje kroničnega bolnika v ambulanti družinske medicine.
