Abstract     The frequency of complications of acute myocardial infarction during first days of treatment was evaluated in retrospective study in 34 patients (PTS) divide into two groups according to therapy. Nitroglycerol infusion 10.4-40.0 microgrammes/min was administered over 4 days to 17 pts, and after that peroral isosorbide dinitrate (3x20 mg) or molsidomine (3x2 mg) until the end of the 15th day (NG group). 17 pts of the control group (C group) were administered 3 x 20 mg of isosorbide dinitrate or 3 x 2 mg of molsidomine orally, over 15 days. The groups were similar according to the patients' clinical characteristics on admission as wll as infarct localization and extent. There was no difference in the survival rate (p = 0.3), and in the left ventricular failure frequency (p=0.1). In the C group higher incidence of ventricular arrhythmias (p=0.005), and higher incidence of infarct extension (p=0.03) was observed. The same group revealed higher incidence of later Q wave enlargement or appearance, and ST segment reelevation or new elevation appearance (p=0.002) in particular. The frequency of total pts with the above mentioned complications was significantly higher in the C group, as compared to the NG group (p=0.0003)
