Abstract		Home environment enabled with interdisciplinary team and its experts work functional enhancement for stroke patients by maximising independence. Team work is individual, goal orientated and is carried out in the long period of time. In the aim of health care rationalisation concept, the result and aim verification, outcome control and cost statement, is of significant importance due to home rehabilitation. To maintain continued rehabilitative process the team must interact constructively, effectively, with regard to patient as team member and with high. manner of knowledge and skills.
