Abstract     Stroke usually happens unexpectedly, changing within a single moment all areas of the patient's experience and behaviour, its consequences also affecting the life the patient's family. Recovery therefrom is an uneven and long-lasting process; some of its consequences remain there for a long time and sometimes for ever. Many factors, originating from the patient himself or from his enviroment, either encourage or impede the recovery. Stroke has a complex impact upon the patient's psyche; the brain lesion directly affects his mental abilities, emotions, and behaviour; his comprehension of the disease and confrontation with its consequences lead to numerous stressful situations. As a rule, stroke also affects his mind while simultaneously playing an important role in the recovery. The changes occurring in the course of treatment are all too often overlooked since they remain hidden from the outside and the patient does not normally report on them. Such overlooked and neglected psychic changes gradually deteriorate, causing ever new problems. Psychological treatment should be involved through all phases of treatment and recovery and also some time after the patient's return to his domestic environment. It is primarily intended for the patient, his relatives and other people from the same environment so that they may have a better understanding of the disease and its consequences, develop and maintain suitable mutual relationships, ensure conditions in which the patient and his family could remain socially integrated in the environment and thereby secure a maximal quality of life. Psychological treatment programs are interwoven with diagnostic and therapeutic elements throughout the process.
