Abstract     1.Background Neurocardiogenic syncope is the most frequent transitional loss of consciousness and posttiral tone. Malignant neurocardiogenic syncope is defined as syncope with prolonged asystole, longer than 5 seconds, during head-up tilt table test ( HUT ). Because of that, there is an opinion of malignant syncope as the most dangerous within neurocardiogenic syncope. 2.Aim The malignant syncope is not well known, that's why we wanted to research what are the similarities and differences between patients with malignant and neurocardiogenic syncope. 3.Hypothesis We expect there are no differences in gender and age between patients with malignant and neurocardiogenic syncope and there is the similar percentage of patients in both groups who have positive HUT in the first 30 minutes of test before using nitroglycerine as provoking drug. We expect the malignant syncope is the diagnosis of patients on another HUT again. 4.Methods We compared the gender, the age of patients and the percentage of patients having syncope before NTG provocation between a group of 616 patients with neurocardiogenic syncope (population A ) and a group of 24 patients with malignant syncope ( population B ). They all had a positive HUT by protocol, previously described by Fitzpatrick et al. We took anamnesis of 18 patients with malignant syncope and invited 12 of them to pass the HUT once again without nitroglycerine provocation. We measured arterial blood pressure with Finapres. We described the data between populations in diagrams and compared the results using mean ` SD and x2 test, at statistical risk p < 0,05. S.Results There are no differences between populations in gender and age. There is much grater percentage of patients with malignant syncope than neurocardiogenic on HUT before nitroglycerine provocation. The sensitivity is higher in nitroglycerine potentiated HUT in both populations. (Abstract truncated at 2000 characters).
