Abstract | Based on extensive evaluation Neonatal resuscitation guidelines have been widely revised in the year 2000. New recommendations concerned temperature and oxygen delivery during resuscitation, mentioned new techniques, like establishing the airway by laryngeal mask, or confirmation of tracheal tube placement by exhaled CO2. The guidelines proposed two thumb-encircling hands for preferred method of chest compression, discouraged the use of albumin, bicarbonate and high dose epinephrine, and simplified time schedule in the new algorithm for resuscitation. Briefly, circumstances where withholdings or withdrawals of resuscitation were mentioned. However, much of what these guidelines proposed was based on the opinions of the experienced clinicians or on less than optimal studies. Even more, the guidelines made no distinction between the techniques for term and very preterm infants. It is very likely that use of cold, dry, 100% oxygen delivered by devices that provide variable inspiratory pressure and tidal volumes may even hurt very preterm infants instead of treating them. | |
