Abstract     Postoperative pulmonary complications are still a significant problem in modern medicine, despite advances in anaesthesia and surgical care. It is now well known that a preoperative pulmonary evaluation and respiratory therapy decrease the risk of postoperative pulmonary complications. ln the paper we present an overview of current recommendations for preoperative pulmonary evaluation and preparation of the patient, who is at risk for postoperative pulmonary complications, with the emphasis on respiratory physiotherapy. A short outline of the current practice at the University Medical Center in Ljubljana, Slovenia is presented. To illustrate the effectiveness of proper evaluation and therapy of a surgical patient with pulmonary complication, a case report of a patient with newly diagnosed obstructive sleep apnoe is presented.
