Abstract     Sudden cardiac death during regular sport activity occurs very rarely. Due to healthy life style and demonstrated favourable impact of regular exercise on body and mind, each sudden cardiac death always strikes relatives, coaches, sportsmen, society and public media. The article deals with a 47-year old sportsman, a jogger and marathon runner, cyclist and mountain climber with an effort-related symptoms appearing after 5-7 km of jogging, that he had not experienced before (unpleasent chest sensations, effort dyspnea). Elevated cholesterol level was the only established risk factor for atherosclerosis. Submaximal exercise test passed by without chest pain or ischemic ECG changes. Coronary angiography disclosed subtotal stenosis of LAD coronary artery in the middle third. The narrowing was dilated by percutaneous transluminal coronary angioplasty and stent deployment. Soon, he regained the same pre-intervention physical capacity and maintains it even after 5 years (marathon race, 200 W exercise test, Kilimandjaro). The case report illustrates an example of an adequate and due diagnostic and therapeutic approach that has not resulted in a sudden cardiac death. The contribution comprises the literature review on the sudden cardiac death during sport activities, causes of death, prodromal symptoms and preventive measures that should be taken.
