Abstract     The basic step in dealing with criticaly ill patient is to open and secure the airway. Endotracheal intubation secures the airway from aspiration and presents the safest method of prevention against aspiration in the prehospital setting. Every medical person working in emergency field should know how to recognize and expect difficult intubation and how to perform the special procedures of opening and securing the airway should the classic endotracheal intubation fail. The clinical examination includes inspection of the patient, his neck and its mobility, shape and size of the upper and lower jaw and submandibular space. We continue with inspection of teeth and throat, we measure different distances on head and neck (thyromental distance, mobility of temporomandibular joints). We also have to recognize the signs of airway compromise (breathing effort, work of the accesory breathing muscles, hoarseness of the voice, intercostal/subcostal/jugular recession, flaring ofthe alae nasi) and we have to perform direct laryngoscopy. These findings help us to timely detect difficult airway and to prepare us to perform special methods to open and secure the airway.
