Abstract		Using a well established laboratory test in a routinely manner we are seldom aware of how it was constructed, of its performance or its limits. Such test can give us a false feeling of certainty or rise concerns over a potentionally life-threatening condition. Patients referred to the emergency department present with a wide variety of clinical conditions. Positive cardiac troponin values that play a key role in diagnosing acute myocardial infarction are frequently detected. The aim of our contribution is to highlight the basic patophysiology of an injured cardiac cell that leads to release of the cardiac troponin. Principles of laboratory tests used for the detection of troponin are explained. This understanding helps us interpret positive troponin values in various clinical conditions.
