Abstract     A 75- year old woman was treated with one oral dose of radioiodine (RAI) because of toxic nodular goitre. Before radioiodine therapy serum thyrotropin receptor antibodies (TSI), thyroid peroxidase antibodies (antiTPO) and thyroglobulin antibodies (antiTg) were negative. Three months after treatment the patient developed severe hyperthyroidism with appearance of TSI. Methimazole therapy was started. Four weeks later agranulocytosis and mild liver failure were found. Methimazole was stopped and the filgrastima (granulocyte colony stimulating factor- (G -CSF)) and antibiotic treatment was initiated. After 4 days neutrophil count and liver tests become normal. A second administration of RAI was found successful. At the follov up she developed hypothyroidism and started with L thyroxine therapy.
