Abstract     Objectives Apart from general therapeutic measures in treating psychiatric patients, special measures to ensure safety, like seclusion and restraint, are used in everyday clinical practice. The natural course of mental disorders, treated in hospital, is often combined with behaviors endangering patients and their environment. Standardized physical safety measures and the use of medication (chemical restraint) to control the accompanying aberrant behavior are used. Among physical safety measures non-restrictive (non-coercive) and restrictive measures are used. Persons with mental disorders, requiring coercive measures are generally treated in psychiatric intensive care units (PICU). Apart from safety measures applied to subjects, admitted to PICU for observation, diagnostics and treatment, long-term antipsychotic therapy is essential for patients with psychosis to prevent relapse. Aims and hypotheses This naturalistic study was conducted to assess the complex field of safety measures used in the treatment of acute psychiatric patients, as well as the problem of treatment compliance. The following hypotheses were proposed: 1. Hypothesis 1: the reasons and number of safety measures may be influenced by various factors like socio-demographic, illness-related, treatment-related, doctrinaire, socio-cultural, environmental, and tradition. 2. hypothesis 2: subjects in psychiatric setting, treated with safety measures differ from subjects without these measures by specific socio-demographic, illness and other characteristics. 3. Hypothesis 3: compliance with treatment in patients with psychosis prior to index hospitalization affects the hospital treatment and secondary the use of safety measures. Noncompliant patients have some socio-demographic, illness-related, and other characteristics not seen in compliant patients. (Abstract truncated at 2000 characters)
