Abstract     Background: Second primary cancers (SPC) are an important clinical and epidemiological problem. The development of SPC is amongst others related also to the continuing exposure to carcinogenic risk factors such as smoking. lt is wetl known that patients with smoking related first cancers have a very high risk of developing SPC. We wanted to investigate the risk of SPC in these patients in Slovenia, the role of smoking in the development of SPC in thcse patients, and the influence of SPC on survival from the primary cancer. Material and methods: We conducted a retrospective cohort trial which included 14 cohorts of smoking related cancers in men in Slovenia, from 1961 to 2000. According to the new data from the International Agency for Research on Cancer (IARC) we included the following smoking-related cancers: lip, oral cavity, nasopharynx, oropharynx, hypopharynx, esophagus, stomach, liver, pancreas, nasal cavity and sinuses, lung, bladder, kidney and mieloid leukemia. We used the person-years approach and expressed the risk for developing SPC as a standardized incidence ratio (SIR). We performed a separate analysis of risk for SPC by follow-up and by calendar period and the reciprocal comparison of SIR for pairs of smoking related cancers. Survival analysis was donc with the Cox proportional-hazards model and relative survival. Survival analysis was limited to smoking-related cancers with better prognosis (lip, oral cavity, nasopharynx, oropharynx, hypopharynx, stomach, nasal cavity and sinuses, bladder, and kidney).
