Abstract     Perioperative care for liver transplant patient is one of the most challenging. Preoperatively patients need carefull evaluation of all organ systems. Degree of liver failure may vary, but the most patients, especially urgent cases, have one or more severe organ failures. Patophysiological changes in patients with end liver failure present with dearrangement of circulation, lung, kidney and brain function. There may be portal hypertension, coagulation and metabolic disorders, changes in pharmacokinetics and pharmacodynamics of drugs and anesthetics. Haemodynamics is monitored invasively. Electrolyte and acidobase balance is often chequed and corrected. Considering specific patophysiological changes the operation can be divided into preahepatic, ahepatic and reperfusion period. The success of the procedure depends crucially on good organisation and cooperation.
