Abstract     Since spleen has been recognized as an immune organ of paramount importance, the ways of treatment of spleen injuries have been thoroughly changed - from splenectomy to preservation interventions and consensus about the conditions of conservative management. In this paper, conservative and surgical methods of treatment have been described, followed by indications and contraindications for respective methods and prophylactic measures in case of splenectomy. A short analysis of blunt spleen trauma in the Clinical department of Traumatology, Ljubljana, in years 2002/03, has been presented. The results demonstrate that more than 50% of patients suffering from blunt spleen injury have been successfully treated conservatively, and that by those treated surgically the total splenectomy was performed in all cases. This fact certainly needs a critical evaluation.
