Abstract     Short historical review of splenectomy is followed by the document of the first successful splenectomy in Ljubljana in 1703. The surgical factic and technique of urgent and elective splenectomy »in situ« are described. The autor's own 15-year series of 78 splenectomies in hematologic patients is presented. Indications for splenectomy, weight of removed spleen (35 out of 78 ones were heavier than 1000 g) and postoperative complications - one severe intraabdominal bleeding and 5 subdiaphragmatic collections are described. One patient with intraperitoneal bleeding had to be reoperated twice, one greater collection was managed with percutaneous drainage. Perioperative antibiotic and antithrombotic prophylaxis were not standard in that time. All patients with collections were treated with antibiotica. There was no postoperative death. One female patient developed a larger incisional hernia which was managed operatively. The long term survival of all these patients is not known.
