Abstract     Background: Breastfeeding problem demands punctual determination what is wrong and why that breastteeding can be preserved. The practice of diagnosis is not done only through clinical symptoms, many women need a more complete assessment. Although many breastfeeding problems reside in the realm of the baby's position on the mother's breast, there is almost always a deeper level of mother's feeling behind it. Conclusions: Four cases with the same symptoms of engorgement but with different histories and assessment of mothers and babies are represented. The importance of history, assessment of mother and baby, formulation of problems, explanation of symptoms, plans for interventions, evaluation of solutions is addressed. The reasons and rationales for using an effective strategies based on looking beyond the surface symptom are also discussed.
