Abstract     Aim: The study objective wasto determine if there is a difference in microbiological results of joint fluids comparing the knee infections that occurred as a spontaneous effusion in the knee joint to those that occurred after the procedures without the implants or after the insertion of implants (reconstruction of anterior cruciate ligament, osteosynthesis). Methods: A total 104 joint fluids were taken from the patients in Jesenice General Hospital from January 1, 2000 through December 31, 2003. During this period, 602 arthroscopy (448 male, 154 female; average age 37 years) and 124 reconstructions of anterior cruciate ligament (92 male, 32 Female; average age 32 yearsj were done. In this period, around 1500 aspirations infiltrations were done in approximately 10 years older population. Results: Eighty-five joint fluids were taken after a spontaneous effusion in the kneejoint, 10 after the injection of drugs, and 9 after the insertion of metallic material. Bacterial pathogens were isolated from 22 joint fluids (11 after a spontaneous effusion in the knee joint, 7 after the injection of drugs and 4 after the insertion of metallic material], and no agentwas identified in 82 effusions. The rate of the positive fluids was lower in the group without the procedures compared to other two groups, the difference being statistically significant (p<0.05). The infection rate between the groups with or without the implant insertion was statistically insignificant Conclusion: We established the incidence of knee jointinfection in different groups of knee effusions in Jesenice General Hospital. In the effusions following a procedure, the incidence af infection was statistically significantly higher, but the extent of the procedure did not affect the risk of infection.
