Abstract     Postpneumonectomy empyema still remains a serious complication, that can result in a lethal outcome. If the cause of the empyema is the bronchopleural fistula, the clinical picture as well as the treatment are much more difficult. In the period of last five years we operated on 427 patients with lung carcinoma. We had to perform 64 (15%) pneumonectomies. Five patients developed a bronchopleural fistula with pleural empyema within thirty days. In these five patients we immediately performed drainage and lavage of the pleural space, started antibiotic treatment and performed a bronchoscopy. Depending on the size of the defect seen with the bronchoscopy we decided on further procedures. Two patients, who had the defect seen on the bronchial stump smaller than 5 mm, were treated conservatively. In the remaining three patients we covered the defect, that was bigger than 10 mm, with the omentum. There were no major complications in the post-operative period. All of the five patients were released. Four of them are still in the outpatient care.
