Abstract     The surgical treatment of the breast cancer evolved into less mutilating procedures in the last three decades. Additionally, in the last decade a new big step towards less invasive surgical treatment of the breast cancer emerged with the introduction of new surgical methods. A guide wire localization is today a standard method of localization of the nonpalpable breast lesions. In our first study, the factors influencing the complete excision of nonpalpable carcinoma were determined when using the guide wire localization. We also found an unacceptably high proportion of non-radical excisions of the nonpalpable breast carcinoma, too heavy average specimen weight and consequently poor cosmetic outcome in our series. A new radio guided occult lesions localization (ROLL) was introduced and the results of ROLL were compared to the hook wire localization in the second study. We found a higher proportion of radically excised non-palpable breast carcinomas, lower average specimen weight, and therefore, better cosmetic outcome in ROLL compared to the hook wire localization. In the third study, we developed a new original method of accurate administration of blue dye in sentinel node biopsy in the nonpalpable breast cancer. The peritumorally injected radioisotope was used simultaneously for the identification of the sentinel lymph node and to guide the excision of the nonpalpable breast cancer. Sentinel node biopsy resulted in a high proportion of reoperations of patients in whom a metastasis was found in the sentinel node, which is an indication for the axillary lymph node dissection. In the fourth study, we assessed the value of the touch imprint cytology as an intraoperative sentinel lymph node evaluation method. (Abstract truncated at 2000 characters).
