Abstract     Primary Sjogren's syndrome (PSS) is the second most common, chronic systemic autoimmune disease, characterized by progressive inflammatory destruction of exocrine glands. The dysfunction of lachrymal and salivary glands, leading to symptoms and objective findings of keratoconjunctivitis sicca and xerostomia is the key clinical manifestation and the mainstay for the diagnosis of PSS. In the past three decades many criteria sets for diagnosing PSS have been introduced as a result of still unknown disease's etiopathogenesis and absence of single, enough specific test to form the basis of diagnosis. Despite recently published classification criteria, proposed by American-European Consensus group in year 2002 (which serve also as diagnostic criteria) the assessment of salivary gland involvement in PSS is still a matter of debate. In addition to accepted tests, namely unstimulated salivary flow test (USF), salivary gland scintigraphy and sialography other methods for confirmation of salivary gland affection are beeing studied. Among them ultrasonography (US) of major salivary glands as noninvasive and nonradiating investigation seems attractive. However in past only few studies evaluated the role of salivary gland US in diagnosing PSS. We aimed to determine with our crossectional study the US salivary glands' parameters that discriminate between PSS and non PSS, and the significance of ultrasonography in the diagnosing PSS. In 218 consecutive patients with suspected PSS, US of both parotid and submandibular salivary glands was performed besides the regular diagnostic procedure following the European criteria proposed by the American-European Consensus Group in year 2002. Five US parameters were semiquantitatively assessed: echogeneity, inhomogeneity, clearness of salivary gland borders, number of hypoechogenic areals, hyperechogenic reflections.
