Abstract     Introduction: Covering a period of 20 years, this retrospective study analyzes various indications for penetrating keratoplasty as well as patient age and additional surgical interventions. The aim of the study was to identify the changing indications in the period of time between 1984 and 1994 and between 1994 and 2004. Methods and patients: Included were altogether 265 eyes of 244 patients, in which penetrating keratoplasty had been performed at the Maribor Teaching Hospital Department of Ophthalmology between 1984 and 2004. All patients were divided in two groups: the first comprised 107 patients operated between 1984 and 1994, and the second comprised those operated between 1994 and 2004. Results: The most frequent indications in all the years together were corneal scars (22.6%), keratoconus (22.3%) and bullous keratopathy (21.9%). Between 1984 and 1994 the most frequent indications were corneal scars (30.8%), keratoconus (23.4%) and corneal dystrophy (21.5%). Between 1994 and 2004 the three most frequent indications were bullous keratopathy (30.4%), keratoconus (21.5%) and corneal scars (17.1%). In the first decade (1984-1994), 44.9% of our patients were aged between 20 and 39 years. In the last decade (1994-2004), 48.95% were aged between 60 and 79 years. In the last decade, the percentage of additional surgical interventions increased from 16.8% to 25.3%. The percentage of rekeratoplasties increased in the last decade as well - from 2.8% to 11.4%. Conclusion: In the last decade the main indication for penetrating keratoplasty is bullous keratopathy. The maj ority of patients belong in the age group of 60-79 years.
