Abstract     Carpal tunnel syndrome (CTS) results from compression of the median nerve in the carpal tunnel. In order to set the CTS diagnosis we use the anamnesis, objective findings and neurophysiological investigation. A complex of symptoms encompasses paresthesias and decreased sensitivity in at least two of three first fingers accompanied by pain in the wrist and radiation above it. Provocative tests are used for indefinite cases or as an additional diagnostic tool for CTS. There are several neurophysiological protocols for CTS verification, but most of them include the measurrement of the median and ulnar nerve distal latencies and comparrison of orthodromic palmar latencies of both nerves. The contemporary diagnostic approach to CTS is of significance in the differential-diagnostic sense und for determinaton of the type of its treatment.
