Abstract     Pulmonary resection remains the most effective treatment for carcinoma of the lung and is associated with excellent long-term survival. Despite this knowledge, numerous arguments still exist regarding the solution of stage N2 non-small cell lung cancer (NSCLC). In 46 patients operated between 1990 and 1995 at the Maribor Teaching Hospital Department of Thoracic Surgery, the N2 stage of disease was first established intraoperatively. Based on experience, survival rate calculations and an increase in life quality of the operated patients, we are in favor of an aggressive approach to the solution of N2 lung tumors, particularly in those patients in which radical resection of the lung and ipsilateral mediastinam lymph nodes is possible. This retrospective review seeks to assess the effectiveness of surgery in these circumstances and to identify the prognostic factors.
